Information, Forms, & Policies
Please Bring the Following to Each Appointment
- A complete list of medications, including vitamins, herbal supplements or over-the-counter medication.
- Your insurance card and copay.
- An "Outpatient Info-Consent to Treat" form (See "Patient Forms" below)
- Before each GYN visit or before your First OB visit, please download and fill out a "History Form" (See "Patient Forms" below).
- New patients, please arrive 30 minutes early to complete paperwork and have your medical records transferred to our office before your first visit. You can print out a "Medical Records Release" form to send to your previous physician. (See "General Forms" below)

Appointments and Phone Calls
Please call 704-316-5270 and press option 3, Monday through Friday from 8 a.m. to 4:45 p.m. to schedule. We require a 24-hour notice for cancellations.
Emergencies
For a life-threatening emergency, dial 911. If you have an emergency or go into labor, call our office at 704-316-5270 and press option 1. The on-call physician will be contacted. If the emergency requires you to seek treatment immediately, please proceed to labor and delivery or the emergency room at your designated hospital. Please contact our office as soon as possible.
Prescription Refills
To refill a current prescription, please call your pharmacy and they will contact us if necessary. Allow one working day for refills. The on-call physician after office hours cannot refill medications.
Lab Results
You should receive notification of lab results by phone or mail. If your results require immediate attention, we will contact you by phone. You should receive notification of a normal Pap Smear within 2 weeks of your visit. You will be contacted by phone if your Pap Smear is abnormal.
Medical Questions and Concerns
We have a registered nurse with many years of experience who triages calls everyday from 8:30 a.m. to 4 p.m. This registered nurse is available to address non-emergency questions. All calls prior to 4 p.m. will be returned the same day. Calls received after 4 p.m. will be returned the next business day.
