Midtown Obstetrics & Gynecology has always been highly invested in protecting patient confidentiality. The Health Insurance Portability and Accountability Act of 1996, known as HIPAA now mandates that we clarify how we utilize your patient health information (PHI).

Your medical chart contains your health history, current symptoms, examinations and test results, diagnoses, treatment and a plan for your future care or treatment in the form of recommendations. It will also contain testing results and any necessary information from a hospital and or any other healthcare provider that we have obtained records from in order to provide you the best of medical care.

Midtown will give your personal health information, including medical history and all test and lab results directly to the birthing hospital, and other specialists needed for your care the baby’s care, if applicable. For surgery, your history and physical and copies of any other related tests will be given to the hospital. These other health providers may make your PHI part of their medical record.

The following are possible usages for your medical information for treatment, planning and operations. At all times, only the minimum necessary information will be utilized:

  • As a basis for planning your care and providing treatment which may include calling in prescriptions and/or refills to the pharmacy of your choice
  • As a means of communicating with other healthcare providers who contribute to your healthcare
  • As a legal document of the care we deliver to you as our patient
  • As a means of documenting to third party carriers (your insurance company)
  • As documentation that you actually received care for services being billed
  • A source of information for public health officials charged with improving the health of the regions they serve
  • A tool for continuity of care and to assess the appropriateness and quality of care you received
  • A tool for identification of specific programs that may be beneficial to you

Release of your health information for purposes other than listed above will require a separate Patient Consent and Authorization signed and dated by you.
